The close-out visit (COV) cleanly closes a site once the last patient is out and the final data are collected. Done well, it protects the study: a site closed by the book is a site ready for an audit or inspection, even months later, in line with Good Clinical Practice (ICH E6).

What close-out verifies

  • Resolution of all outstanding data (queries, deviations, open points)
  • Completeness and consistency of the investigator site file (ISF) and the TMF
  • Final investigational product accountability, with documented returns or destruction
  • Handling of study samples and equipment
  • Archiving obligations and durations, understood and organised by the site

Why it is never rushed

A hurried close-out leaves gaps that resurface at the worst moment: a query never resolved, a missing document, unclear archiving. The close-out visit is the last chance to fix things while the team is still engaged and the study is fresh in memory.

How I work

I run a complete close-out checklist, make sure every open point is resolved or traced, and leave the site with clear archiving instructions. You get a site closed with no documentation debt.
